Initial Consultation

Service Description

During the initial consult we will review your current complaints, past injuries and medical history; assess lifestyle and social factors that affect health; and establish goals. Appointment will be schedule upon receipt of completed paperwork. Labs may be ordered prior to or after appointment. Fee includes doctor review of records and labs. Please provide copies of any relevant imaging/reports & labwork, along with intake forms.
